Do you take my insurance?
Dr. Reams Concierge PT is an out of network provider. Cash is required for services rendered, however if requested a “super bill” (receipt with billable codes) will be provided. You can submit this document to your insurance for reimbursement depending on your plan.
Where do you treat me?
Dr. Reams Concierge PT comes to you. This can be your home, office, gym (if allowed) or anywhere legally allowed. Dr. Reams is licensed in Tennessee and Georgia so all treatment must be provided in Tennessee or Georgia.
What if I have Medicare, can you treat me?
Dr. Reams Concierge PT is not allowed to treat patients with Medicare insurance even if it is secondary. Medicare has very strict rules regarding this.
How many times will I need to be treated?
This depends on what you need help with. Kalie will evaluate you at the first visit to determine necessary treatment. She will then have an open conversation with you about a recommended plan of care. Typical plans of care range from 3 to 12 on average.
Do I need a note from the doctor?
Initially Kalie does not need a note from your doctor to treat you. However if treatment lasts longer than 90 days a note is required. (this should not happen, as you should get better by then)
